Lees-McRae College Health Care Plan

Full-time employees and their dependents are eligible to participate in this plan on the first of the month following, or coinciding with, completion of 30 days of active service, which is the required waiting period.
The College pays approximately 80% of the premium for the employee; however, the employee is responsible for the cost of dependent coverage.
This is a closed enrollment plan which means there is no annual enrollment. You must enroll in the plan within your first 30 days of employment or you will no longer have the option to enroll barring a qualified life event.
- Deductible: Individual - $1000.00, Family - $2000.00 (Based on calendar year)
- Co-Pay: $30.00 fixed amount for general physician office visit (limited to $200.00 charge)
- Drug Card Co-Pay: $60.00 name brand/$10.00 generic
- Preventive Services such as annual physicals, pap smears, and mammograms are covered 100%.
Mail Order Prescription Plan
Prescriptions may be obtained through the mail.

Lees-McRae College provides life insurance to all full-time, permanent employees. The benefit amount is equal to the employee's annual salary. $2,000 Dependent Life coverage is also provided by Lees-McRae College at no cost to employees.
Dental Insurance is offered by Guardian Insurance Company. Open enrollment takes place in December for the following calendar year. The employee pays full cost; however, the premium may be pre-taxed.
Membership is available in the Truliant Federal Credit Union. Contributions are made through payroll deduction.

Payroll direct deposit is available to all employees.
The Direct Deposit Agreement Form should be completed and returned to the Human Resources Department with a void check (for checking account) or a deposit slip (for savings account).
Flexible Spending Account - Section 125
Employee reimbursement accounts, known as Flexible Spending Accounts are available to Lees-McRae College full-time employees. You may allocate part of your earnings on a pre-tax basis for the purpose of reimbursement for certain out-of -pocket medical and/or day care expenses. Open enrollment in the plan is once per calendar year during the month of December. The Flexible Spending Account runs on the calendar year.
Long Term Disability Insurance
Long Term Disability insurance, also known as LTD, is provided to all full-time, permanent Lees-McRae College employees.
The coverage is with Guardian Insurance Company. The elimination period is 91 days for illness before benefits begin. The monthly benefit is 60% of covered monthly earnings not to exceed a maximum monthly benefit of $6,000 which will be reduced by other income benefits. An employee's covered monthly salary is based on the previous year's W2 statement.

Tuition waiver benefits for credit classes are available for full-time employees and their dependents. Requests for tuition waivers must be submitted to the Director of Human Resources prior to the semester.
Vacation, Sick Time, Paid Holidays
Vacation days, sick days and paid holidays are provided based on the employee's job classification. Further information may be obtained in the Employee Handbook, Section 5.06 through 5.12.
529 College Savings Plan
Employees may allocate a portion of their earnings as a method of saving for the cost of education for themselves or their dependents. Interest earned on this money is tax-exempt when used for educational purposes. Minimum contribution - $5 per month. The program is administrated by the North Carolina College Foundation.
If you are injured on the job you should:
- Report the accident to your supervisor immediately. ( Supervisors are responsible for reporting such injury to the Office of Human Resources.) Incident Report Form
- If medical treatment is required, promptly go to one of the following facilities:

Referral to other medical providers must be approved by the Human Resources Office or the Workers' Compensation Insurance Carrier.
Note: Cannon Memorial Hospital should be used only when there is danger to life and limb or when the Davis Health Services is closed. Medical care obtained without permission may be at your own expense.
Be sure to inform Cannon Memorial Hospital personnel that medical treatment is for a job-related injury or illness and should be filed as "Workers' Compensation". Employees injured on the job will be required to take a drug test at the time of injury.
TIAA-CREF Retirement Plan
All full-time employees are eligible to participate in the College retirement plan after one year of employment. The waiting period is waived if the employee was actively participating in a 403b plan prior to employment at Lees-McRae College. The employee contributes 1-5% of his/her gross salary which is matched by the College.
